Three Levels of Personal Assistance
Keystone Meadows offer three levels of personal assistance ranging from minimal assistance to full assistance. Our Executive Director and Director of Health Services, along with the input of each resident, family, and physician build a personal service plan that will best meet the needs of each resident. We expect all of our licensed nurses and certified staff to deliver nothing less than the highest quality personal care and services for our residents. Developing the right service plan for each individual is important and that is why we are here. We want to make each day enjoyable.

I love my moms spacious sunny apartment. It is so roomy. The meals are delicious and varied. Activities are so enjoyable and something different all the time. Everyone here is familyhelpful, happy and very attentive. I love for mom to have her little dog Peteau with her. The staff is very good to provide transportation to and back for moms medical appointments. Oh, yes, they decorate for every season and holiday. It really keeps us in a pleasant state of mind. All the staff is exceptional as well.

Personal Service Plans
Personal Service Plans are developed for each Keystone Meadows resident to ensure comfort and well-being while maintaining dignity and choice. The Personal Service Plans are developed upon admission. When going through an assessment for the first time, families should be prepared to provide information on the residents prior life, customary routine, preferences, etc. Using the personal service plan approach, families have a better idea of the needs of their loved one. Some of our residents need minimal assistance, while others require more assistance. At Keystone Meadows, our staff provides ongoing assessments to ensure the well-being of each resident. Residents and family members are kept informed regularly of changes in needs as assessed by our staff. You only pay for the services you need.
Each Residents particular needs are scheduled on a daily basis, therefore we all know what to expect. Our nursing department maintains records of each residents service plan as well as other health information. You only pay for the level of service rendered.
