The Maplewood Transitional Care program at Keystone Place at Newbury Brook provides a compassionate, secure, and enriching environment for individuals in the early stages of dementia or memory impairment. This unique setting offers independence during the day and added support and safety at night, supporting each resident’s journey with dignity.
Each day at Maplewood is designed with intention, supporting the whole person through:

Emotional Support – Compassionate care for residents and reassurance for families
- Private apartment-style living
- Daytime freedom in a non-secured environment, secured setting at night
- Access to individualized personal care services (i.e., bathing, grooming, dressing, eating, toileting, ambulation, etc.) and medication management
- Health and wellness monitoring by a licensed nurse
- Trained care associates available to assist with daily routines 24/7
- Three (3) meals per day with emphasis on nutrition, MIND Diet selections that promote brain-health
- Emergency response system
- All utilities (except personal phone service)
- Weekly housekeeping, linen and personal laundry service
- Scheduled transportation to medical appointments and recreational outings
